Please note that this form is only a request for an appointment, not a confirmation.
The doctors and staff wish to provide you with the highest quality of dental care. Because it is in the best interest of
the patient, it is important to us that you receive the time and attention that you deserve at each visit. The doctor, the
doctor’s assistant or the registered hygienist, and their support staff make every effort to honor scheduled
appointment times, we ask that our patients please do the same.

In the interest of maintaining the fairness and integrity of our appointment system, and to assure the availability of
appointments to our patients for necessary treatment, we require a minimum of 48 hours
advance notice if you are unable to use the time that has been reserved for
you.

Patients that cancel or reschedule their appointments without proper notice will be assessed a $50.00 per hour
charge to offset lost time, effort, and materials that the staff has spent preparing for your appointment.
